Scope

This policy covers information entered by children, young people and adults into the myView Questionnaires app for processing and storage.

Personal Information collected

The myView Questionnaires app is for the collection and storage of information for specified projects.

The information may be about your experience of particular services or aspects of your wellbeing to develop services for groups or individuals. You may be invited to provide demographic data such as age, gender, education level.

The information you provide is stored securely in unique online project databases. The personnel who are authorised to access information are identified in the consent form.

You may access the app anonymously or with a unique identifier login

• If you access the app anonymously no personally identifying information is recorded about you.
• If you have a unique identifier this may or may not be linked to other information held elsewhere about you.
• If your unique identifier is associated with other information about you, this will be detailed precisely on the consent page at the beginning of the app. Having read this information, you can choose whether or not to continue to use the app and provide information.

f you choose to continue you have a choice about whether or not to answer a particular question.

Once you have provided information, you are entitled to withdraw your consent. If you do so,  any information you have provided will be deleted

Consent

You have a choice about whether or not to provide any information. Before entering any information, you have to confirm that you have read and understood the myView Questionnaires app Privacy Policy and consent to provide information.

After providing your information you have the option to withdraw your consent.  If you withdraw your consent any information you have provided will be deleted.

Data Security

Individual responses are encrypted and transmitted securely using https from the app to a secure database. The information you provide is only available to personnel authorised to access the project data and for the purpose specified.

Information provided is not shared with any other parties. Data is only retained during the duration of a project. At the end of a project your data is deleted.
